Step 1: Completing Your Intake Forms
The first step in the process of setting up your trust is to complete the intake form. They are multiple pages to complete and we don’t expect you to do it all in one sitting. Carve out a little time to sit down with the forms, don’t get overwhelmed, and please let us know if you have any questions!
You have two options:
Option 1: Fill Out the Web Form Online (Preferred)
The first and preferred option is to complete the form via our secure Form Builder (Platoforms).
The form will allow you to save and complete it at a later time. At the bottom of each page of the form is a link to “Save for Later”. An email will be sent to you with the link that you can return to at any time. If for some reason you don’t get the email please check your spam folder, or you can always reach out and we can get it to you.
Once you have completed the form, click “Submit” on the last page and it will send us a notice. However, we’d love it if you’d also just drop us a note via text or email.

Option 2: Download the Form by PDF
If you prefer to download the form by pdf, you can either print and write out your answers by hand or complete the pdf on your computer. If you write them out by hand then you would need to scan or snap photos of all pages. If you fill out on the computer please ensure that you have downloaded and saved the pdf to an alternate location and know how to find it so it is being saved along the way.
